Step 1: Go to https://castcertificatewb.gov.in/application_ews
Step 2: Choose EWS applied for State or Central.

Put the following in Application to section - 
Select District
Sub-Division
Municipality
Block GP/Ward

Put the following in Application for section - 
Name
Father's Name.
Mobile.
Email

Put the following in Document section - 
Aadhaar No
Khadya sathi Category
Khadya sathi No

After that fill up the Date of Birth, Place of Birth, District, Police Station, Village/Town, and then put various information in Address page, Referee Details page and declare Income & Asset details and submit the application.
